Report Directions
- Please include your grant reference number on the report and mail it to: Attn: Grants Administrator, Gill Foundation, 2215 Market Street, Denver CO 80205
- Please do not exceed four pages, excluding attachments
- Progress and/or final reports are due with next proposal to the Gill Foundation or, if no further proposal is submitted, thirteen months after the date of your grant award letter as indicated in your award letter.

Please separately answer the following questions:
- Did you achieve, or are you on schedule to achieve, the goals, benchmarks, and/or outcomes proposed in you grant proposal. If not, why not?
- Did you follow the plan and strategies outlined in your grant proposal? If not, why not?
- As it relates to achievement of the goals, benchmarks, and/or outcomes outlined in your proposal, what strategies worked? Why did they work?
- As it relates to achievement of the goals, benchmarks, and/or outcomes outlined in your proposal what strategies did not work? Why didn’t they work?
- Did you raise the remaining funds as proposed for this work? If so, from whom? If not, why not?
- If your organization experienced changes in organizational structure or capabilities as a result of the grant, please explain.
- If you received a challenge grant, did you meet the challenge? (If not, how much did you meet? What were your barriers?) As an attachment, show how you met your challenge parameters, including the number of donors and dollar ranges of cash gifts that the Gill Foundation matched. If we matched foundation or corporate gifts, please name the funding sources in an attachment.
- If there are anomalies in your financial attachments, such as an operating deficit or large surplus, please explain.
Attachments
Please include the following attachments with your report. No other attachments are allowed.
- Actual income and expenses to date. If you had a restricted grant for a particular project we are interested in the project budget as well.
- Current year budget including income and expenses. If your budget changed during the year, include the original budget and explain the reason for the change.
- Previous year financials (audited, if available) including income and expenses.
- If you were funded to prepare a report or publication, please include it.
- If you received a challenge grant, please include the attachments as requested above.
